Annual Survey

The annual survey is for community museums that are registered or accredited in the History Trust's Community Museums Program. The survey gathers information about the activities, needs and collections of community museums as well as volunteering and visitor statistics.  

If you require a copy of your survey after you have completed and submitted the form, please contact us and we will email you a WORD document version.
